Mo te tunga | About the role

The registered Social Worker responds to the psychosocial needs of turoro/patients and their families who come into Hospice care. As part of the Family Support team of allied health professionals and wider multidisciplinary team, you will:

• Maintain your own caseload of referred patients and provide dedicated clinical care
• Play an active and collaborative role in the multidisciplinary team
• Provide evidence-based social work that complies with relevant legislation and policy standards
• Build and maintain links with community agencies
• Contribute to internal and external training and education sessions
• Facilitate groups

This is a Casual position with no set hours, to provide cover as needed for team annual or sick leave, working across all our sites.

He korero mou | About you

You have demonstrated experience in social work, and experience or a genuine interest in palliative care and knowledge of the holistic needs of patients and their families. Our ideal candidate will have:

• A bachelor’s degree in social work
• Registered with the Social Workers Registration Board (SWRB)
• A valid, clean full NZ driving licence
• A strong interest in the local community
• Experience working in healthcare (preferred)
• The ability to work autonomously while also contributing effectively to the multidisciplinary team

You’ll be empathetic, an active listener and have great communication skills with the ability to adapt these to a wide-ranging audience. You’ll be a team player who is happy to pitch in and help where needed and have great time management skills with a flexible attitude. Networking and group facilitation skills would be a great advantage.

Harbour Hospice is one of the largest specialist palliative care providers in Aotearoa New Zealand, caring for turoro/patients and their whanau across the North Shore, Hibiscus Coast and Warkworth/Wellsford regions. We support and provide specialist palliative care to people in their homes, in one of our two inpatient units, or in community programmes offered at each of our three sites.
