About the Role
Advance your clinical pharmacy career in a dynamic and patient-centred environment. This is a unique opportunity to lead and shape pharmacy services within the expanding Hospital in the Home (HITH) programme at Te Toka Tumai Auckland. Join a forward-thinking team delivering acute care in community settings, improving patient outcomes and reducing hospital admissions.
As a Senior Pharmacist in the HITH service, you will:
- Provide expert clinical pharmacy services for patients receiving hospital-level care at home, including intravenous antibiotics, anticoagulation, pain management, and other complex therapies.
- Collaborate closely with HITH physicians, nurses, and allied health professionals to ensure safe, effective, and patient-centred medication management.
- Champion medication safety by clinically reviewing prescriptions, monitoring therapy, and supporting transitions of care between hospital and home.
- Lead the development and refinement of protocols, treatment guidelines, and medication administration procedures tailored to the HITH setting.
- Mentor and educate pharmacist colleagues and the wider multidisciplinary team, sharing your expertise in community-based acute care.
- Contribute to service evolution: Play an active role in shaping the future of HITH pharmacy services, including digital health integration, patient education, and expansion into new therapeutic areas.

About you
We are seeking a proactive and patient-focused clinical pharmacist with a passion for innovative care delivery. You demonstrate initiative, adaptability, and excellent communication skills, and align with the Te Toka Tumai values.
Essential:
- Bachelor of Pharmacy (B.Pharm) or equivalent qualification recognised in New Zealand.
- Registered Pharmacist with the Pharmacy Council of New Zealand (or immediately eligible for registration).
- Hold a current Annual Practising Certificate (or be able to obtain one).
- Strong clinical pharmacy foundation, typically gained through at least 3 years of hospital pharmacy experience.
- Excellent communication, time management, and relationship-building skills.
- Strong understanding of medicines governance and medication safety principles.
- Ability to work effectively both autonomously and within a multidisciplinary team.
- Commitment to working effectively in a bicultural environment and upholding the principles of Te Tiriti o Waitangi.
Desirable:
- Five or more years clinical pharmacy practice with experience in ambulatory care, or community-based clinical pharmacy.
- Postgraduate clinical pharmacy qualifications (e.g., PG Diploma in Clinical Pharmacy, Masters degree).
- Pharmacist Prescriber highly desirable, or a commitment to obtaining this qualification.
- Demonstrated project leadership experience, particularly in the design, implementation, and evaluation of new clinical services.
- Familiarity with digital health tools and remote monitoring technologies.
