Join a forward-thinking team delivering acute care in community settings, improving patient outcomes and reducing hospital admissions.As a Senior Pharmacist in the HITH service, you will:Provide expert clinical pharmacy services for patients receiving hospital-level care at home, including intravenous antibiotics, anticoagulation, pain management, and other complex therapies.Collaborate closely with HITH physicians, nurses, and allied health professionals to ensure safe, effective, and patient-centred medication management.Champion medication safety by clinically reviewing prescriptions, monitoring therapy, and supporting transitions of care between hospital and home.Lead the development and refinement of protocols, treatment guidelines, and medication administration procedures tailored to the HITH setting.Mentor and educate pharmacist colleagues and the wider multidisciplinary team, sharing your expertise in community-based acute care.Contribute to service evolution: Play an active role in shaping the future of HITH pharmacy services, including digital health integration, patient education, and expansion into new therapeutic areas.Please see the position description hereAbout the Team/Service/ LocationTe Whatu Ora Te Toka Tumai Auckland delivers health and disability services to over half a million people.
